Are we talking about this form needing notarized?  Or a different one??

My thoughts are that perhaps we could have the important parts filled
out beforehand, show up, and then sign the forms in front of Annaleigh.
Also, will she be charging for this service?  If so, we may have to hold
off, because even getting to the practice may tap us out until I get my
next check. :(
