Hi Derek,

Are you seeing this problem with the latest OpenSlide nightly build?  I 
just tried your test program with:

- 32-bit Windows 7
- Visual Studio 2010 Express
- Release (+ /OPT:NOREF) and debug builds
- OpenSlide 20140123 nightly build
- CMU-1.svs and CMU-1.mrxs

and could not reproduce your issue.
