i don't know all the answers, but here is something to get you further

also got confusing messages from saslpasswd2 in freebsd, don't know why,
everything seems to work ok
( in my case the message is: "error deleting entry from sasldb:
DB_NOTFOUND: No matching key/data pair found" )

try this:
"cyradm -user cyrus --auth digest-md5 localhost imap"

this is good, admin users, like cyrus, should not have mailboxes

When you can login as cyrus in cyradm, then you can create mailboxes just
like you tried to before.
