[SCA-Dance] Rendance Survey

To those who ran into the "illegal choice" error-- thanks for
reporting it.  It has been fixed.  There was a bug causing this to
appear if you included the subject option mentioning "Commedia
dell'Arte".

There are lots of interesting responses coming in, many from sca-dance. Thanks!
