Reading file exported from Aperio ImageScope

The problem is in openslide-vendor-aperio.c at line 358. When adding
associated images, it assumes that if it has only seen one level, then the
associated image is the thumbnail. So when there is only one level, each
associated image in turn is put into the thumbnail slot.

I guess it is written this way because the image description for the
thumbnail is in a different format than the image description for the other
associated images, and the parsing in add_associated_image() will not work
for the thumbnail. I'm not sure what the fix might be. Perhaps the parser
in add_associated_image() could be smarter, or the default at line 358
could be taken only if there is not already an associated image.
