Gary,

I had the same problem, so set unixhierarchysep: no and it persisted. I
manually created the mailbox in cyradm with

cm user.john.#calendars.Default
sam user.john.#calendars.Default jwrm all cyrus all

and that made it work with TB. I've not had time yet to work out whether
I have been overgenerous with the permissions, but I got it to work.

I think the bug here is nothing to do with the separator, but that the
calendar mailbox is not auto-provisioned. I see nothing in the logs to
indicate an attempt at autocreation.
