cannot authenticate to Sieve

Hi,

I have been following several threads concerning similar issues, but 
could not find the information which would have helped me solve the 
issue I am encountering. When using sivtest as follows:

Which settings are missing or wrong which could cause this? It seems 
some information on how to access the MySQL database is missing, but 
I do nto know which.

Any help would be greatly appreciated.
