createmailbox: Permission denied - again ?

Hi All,
I’m having a problem with Cyrus that seems to haunt me from time to
time. When logging in using cyradm to create accounts, I’m told
“createmailbox: Permission denied“. The strange thing is that it seems
to work sometime and then not others … I’m very confused about this (to
say the least):
I’ve read many posts that suggest that the user is not listed in admins:
in /etc/imapd.conf – my user is.
I’ve also read posts that suggest that it has something to do with file
permissions … however this side of things also seems fine.
Here are some examples of what I see. The first thing that confuses me
is why I’m prompted for a login and password twice (see below).
[root at livemail1 etc]# cyradm --user admin at domain.com --auth PLAIN localhost

createmailbox: Permission denied

as you can see I’ve created two mailboxes in here before
localhost> lm
user.chowson (\HasNoChildren) user.wreuvers.Sent (\HasNoChildren)
user.wreuvers (\HasChildren) user.wreuvers.Trash (\HasNoChildren)
user.wreuvers.Drafts (\HasNoChildren)
running imtest seems to indicate that things are fine (as far as I can
tell anyway)
