Your sasldb is in /etc/sasldb or /etc/sasldb2, right? Is the file readable
by user cyrus?

Forget testsaslauthd, because you are not using saslauthd!

Is there only deliver, or also a deliver-wrapper which runs as user cyrus?
The user/permissions could also be a problem here.
