cyrus-imap and full ldap support

well, the way we're doing it the autocreate patch from UoA allows us to 
not need to manually add users into the mailboxes.db as you say.  So we 
create the user in ldap then their mailboxes.db entry gets created.

I haven't worked deleting out yet.  hah.
