Solved: DBERROR db4: fatal region error detected

in my case it turned out to be one more db that I'd forgotten about. 
tls_sessions.db was still leftover from a previous installation. Removing 
this as well solved all my problems. The posixmutexes were just a red 
herring ...

SO: if you move from one version of BDB to another, make sure that you 
either delete all files (if they aren't important) or that you use db_dump 
and db_load.
