Did you set, 'unixhierarchysep: yes' in your imapd.conf ? :-) If you did, it's just a matter of 
creating the mailbox using: user/fname.lname at domain.tld  (the @domain.tld assumes you are using 
cyrus-imapd >= 2.2).
