What are the unixhierarchysep and altnamespace options in imapd.conf set
to (if anything)?

It probably makes more sense to use:
if address :domain ["Sender","From"] "whs1.tow.net"
> fileinto "INBOX.backup";}

> fileinto "INBOX";
^^^^^^^^^^^^^^^^^
You could just use 'keep' rather than fileinto 'fileinto "INBOX"'
