Verification error...

I'm fighting with Postfix - MySQL - Cyrus-IMAP and Cyrus-SASL.  Everything works perfectly
well when I send mail locally (via telnet to postfix) without verification and it also
works perfectly good with IMAP and POP3 access.

But when I try to send mail from my Outlook to Postfix with SMTP-AUTH (same login and pass
as POP/IMAP) I get this in the log;

I don't get how I can get more than one result from pam_mysql?  Anyone got any ideas?
