I have a similar problem, FreeBSD 5.3, Cyrus 2.2.10.

Is this a clue, tried to use saslpasswd2
stranger saslpasswd2: setpass succeeded for cyrus
stranger saslpasswd2: Couldn't update db

The logs says everything is fine...
