Well, since the number of LIST operations on an IMAP server swamps the
number of updates, for 2.0 the only option is flat.

Note that flat is indeed a terrible format for systems that need larger
numbers of updates, for them I recommend upgrading to Cyrus 2.1 and using
skiplist.

Actually, I recommend that to everyone.
