This is making a few assumptions (such as your using cyrus 2.1.x), but above 
you listed sasldblistusers. Are you using /etc/sasldb instead of /etc/sasldb2 
or sasldblistusers instead of sasldblistusers2. Also run cyradm like this:

cyradm --user cyrus --server localhost --auth CRAM-MD5
