[SCA-AE] question regarding new release forms for models, photographers and contributors...

Are separate forms required for separate websites and publications, or 
is there a central registry where these forms are held? In other words, 
if I want to allow a photograph of me to be published in my local 
newsletter, the kingdom newsletter, my local website, the kingdom 
website, any order website, the scribal website, etc. etc., do I have to 
send in a separate model release form to each chronicler and webmaster? 
If not, where do I send the form to ensure that the release (or news of 
it) percolates to all involved?
